Immunization Program
- Children (birth to 18): Must meet the Vaccines for Children (VFC)
guidelines: Medicaid enrolled, Has no insurance, American
Indian/Alaskan Native, Underinsured. Donations accepted. Call for
clinic schedule.
- Adults: Fee varies. Call to schedule an appointment.
- Immigration: Fee varies. Call to schedule appointment.
- Travel: SDHD is a designated Yellow Fever site. Fee varies. Call
to schedule an appointment.
Tuberculosis Program
- TB testing: $10.00. TB testing conducted any weekday except
Thursday.
- TB Education and referral: No charge
- TB medications to Woodbury County residents: No charge
Breast and Cervical Cancer Early Detection Program
Women ages 50-64 who are uninsured or underinsured and meet
the program income requirements may receive the following services
at no charge. Call for more information or to enroll.
- Mammogram
- Pap Smear
- Clinical Breast Exam
- Limited Diagnostic Follow-Up
Senior Health Program
Woodbury County residents age 60 and over who have not been
hospitalized or seen by a physician in the past year may receive
the following at no charge. Donations are accepted to defray
costs. Call for an appointment.
- Nursing Physical Assessment
- Medication Review
- Referral for Further Care
- Counseling
- Limited Lab Studies
Home Health Program
Woodbury County residents who meet the agency admission
criteria with a physician's order. Intermittent Skilled Nursing
Services (24-hr call)
- Intermittent Skilled Nursing: Third party billing/Sliding fee
scale
- Homemaker/Home Care Aide: Third party billing/Sliding fee scale
- Psychiatric Skilled Nursing (Psychiatric Diagnosis): Third party
billing/Sliding fee scale
HOPES (Healthy Opportunities for Parents to Experience Success)
Individuals/Families at risk who are pregnant or have an
infant under the age of 4 months at the time of enrollment
- Voluntary home visits by an RN
- Education
- Support
- Nurturance
